SYNChronicity convenes clinicians, service providers, government officials, community-based organization staff, fiscal managers, consumers, and youth, among others, to address HIV, HCV, and LGBT health through education, training, networking, and resource sharing. Together, participants “SYNC” across systems, data, programs, models, interventions, and policies to create targeted solutions within a dynamic health care environment.
